Starz Cheer and Dance Contract

By being on the Starz Cheer and Dance

I hereby release and absolve Starz Cheer and Dance Organization, its subsidiary, staff, employees, and directors from all liability and responsibility from injuries, sickness, accidents, loss of money and property, that may be sustained while participating at a Starz event, workshop, camp, practice, recital, class or competition. I also hereby acknowledge the risk involved when taking part in cheerleading and/or dance, and have the necessary insurance to cover such, in the unlikely event of an injury, accident, or loss when taking part o Starz Cheer and Dance Co. Inconsideration of me to be legally bound and agree to waive and release all rights to claim for damages which I or my child at sustain or suffer while participating at the event, including traveling to and from an event. I also confirm that I/my child, have not be advised by a doctor or paramedic to avoid physical exercise and do not know of any problem that adversely effect my/their health when taking part at the event. I also give permission for myself/my child to be photographed, video or audio taped during any event and give permission for such photographs, videos and audio tapes to be used in print, promotion and publicity.
